SOCIAL CARE CASE MANAGEMENT SOLUTIONS FRAMEWORK UPDATE

A briefing paper is now available providing an overview of the Social Care Management Solution Framework and the key procurement activity and tender timescales.

 

The Social Care Case Management Solution is a Digital Office framework managed by Scotland Excel which can deliver a self-directed support capability for citizens to improve the impact that care and support has for them. This will allow citizens to have the ability to choose and control what type of social care services they receive, where and when they will receive their care and who will provide their care.


In conjunction with supporting citizens, the solution will support the digital capability for health and social care professionals involved in the direct delivery of care. They will be able to safely and securely access, update and appropriately share relevant information about the individual being cared for. This will encourage the use of decision support tools and services to support frontline practice, self-management and shared decision making, whilst establishing and implementing the standards required to deliver interoperability and information sharing across different health and care systems.
