Job Opportunity: Project Manager (CHI in Local Government)

In partnership with COSLA, we're building a dynamic team to identify opportunities for design and adoption of common solutions (process, technology, governance) of Community Health Index in Local Government (CHILG).

This initiative is pivotal in enabling the wider use of the Community Health Index (CHI), already in wide use in core NHS systems, as a patient identifier within Social Care, helping to streamline the patient journey between Health and Social Care providers, and improve outcomes in critical areas like hospital discharge.

We're now looking for a Project Manager to help drive this work forward. If you're passionate about digital transformation, collaborative working, and delivering meaningful change in public services, this is your chance to make a real impact!
